  • In a gripping sequel to celebrated novel, Fracture, New York Times bestselling author Megan Miranda once again straddles the line between life and death. When Carson pulled Delaney out of the frozen water of Falcon Lake, he died on the side of the road with his mouth pressed to hers. When Troy tried to recreate Delaney's accident, the lake took him instead. All the talk about a curse doesn't shake Decker, until yet another unthinkable tragedy strikes. There's just too much coincidence and death for Decker to take . . . and too much anger. Because Delaney knew it was coming, and she never said a word. Falcon Lake still has a hold on them both, and Decker can't forgive Delaney until he knows why.

  • At the writing of this book, the United States is in the midst of an intense public debate concerning a widely perceived need for reform of the Health Care Delivery System. The reform is primarily aimed at the provision of medical insurance to a large segment of the population currently deprived of that coverage and to the reduction of the escalating costs of medical care. Solutions to the existing problems have been elusive because the causes of the dilemma are multifactorial, complex, and diffucult to identify clearly. There is, however, general consensus that the use and abuse of technology has played a major role in the growing costs of medical care. The importance of fracture care in the overall financing of the health care reform is significant, since injuries to the musculoskeletal system are responsible for a very large percentage of the general expenditures in this area. The cost is not limited to hospitalization and professional services, but also impacts the economy with tempo rary or permanent interruption of individual productivity.

  • This book has become necessary as a consequence of the rapid expansion of the surgical procedures and implants available for spinal surgery within the "AO Group". We have not attempted to write an in-depth book on spinal surgery, but one which will help the surgeon in the use of AO concepts and implants. We con sider the practical courses held all over the world essential for the teaching of sound techniques so that technical complications and poor results can be avoid ed for both the surgeon and, in particular the patient. This book is a practical manual and an outline of what is taught in the courses. It is intended to help the young spinal surgeon to understand the correct use of AO implants. The indi- tions given will aid the correct use of each procedure. . It must be strongly emphasized that surgery of the spine is technically de manding. The techniques described in this book should only be undertaken by surgeons who are trained and experienced in spinal surgery. Certain techniques, in particular pedicle screw fIxation and cages, have not yet been fully approved by the FDA in the United States. However, throughout the rest of the world, the use of pedicle screws has become a standard technique for the spine surgeon, since it has been shown to improve fIxation techniques and allow segmental correction of the spine. The use of cages has become more and more popular, specifIcally as a tool of minimally invasive spinal surgery.

  • This open access book aims to provide a comprehensive but practical overview of the knowledge required for the assessment and management of the older adult with or at risk of fragility fracture. It considers this from the perspectives of all of the settings in which this group of patients receive nursing care. Globally, a fragility fracture is estimated to occur every 3 seconds. This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.

  • This book offers a collection of 17 scientific papers about the computational modeling of fracture. Some of the manuscripts propose new computational methods and/or how to improve existing cutting edge methods for fracture. These contributions can be classified into two categories: 1. Methods which treat the crack as strong discontinuity such as peridynamics, scaled boundary elements or specific versions of the smoothed finite element methods applied to fracture and 2. Continuous approaches to fracture based on, for instance, phase field models or continuum damage mechanics. On the other hand, the book also offers a wide range of applications where state-of-the-art techniques are employed to solve challenging engineering problems such as fractures in rock, glass, concrete. Also, larger systems such as fracture in subway stations due to fire, arch dams, or concrete decks are studied.

  • In the modem language of reservoir engineering by reservoir description is understood the totality of basic local information concerning the reservoir rock and fluids which by various procedures are extrapolated over the entire reservoir. Fracture detection, evaluation and processing is another essential step in the process of fractured reservoir description. In chapter 2, all parameters related to fracture density and fracture intensity, together with various procedures of data processing are discussed in detail. After a number of field examples, developed in Chap. 3, the main objective remains the quantitative evaluation of physical properties. This is done in Chap. 4, where the evaluation of fractures porosity and permeability, their correlation and the equivalent ideal geometrical models versus those parameters are discussed in great detail. Special rock properties such as capillary pressure and relative permeability are reexamined in the light of a double-porosity reservoir rock. In order to complete the results obtained by direct measurements on rock samples, Chap. 5 examines fracturing through indirect measurements from various logging results. The entire material contained in these five chapters defines the basic physical parameters and indicates procedures for their evaluation which may be used further in the description of fractured reservoirs.

  • Fractures are a common finding in children and it is estimated that 2.1% of all children will suffer at least one fracture before the age of 16. With young children in particular, the question may arise if this is related to child abuse. The aim of this book is to help physicians involved in child abuse cases to interpret radiological findings in light of the forensic circumstances under which they occurred. The authors present up-to-date literature related to the mechanisms underlying non-accidental cases of trauma. In this book not only the radiological findings in child-abuse are discussed, but more importantly, these findings are analyzed from a forensic perspective. Careful attention is paid to evidence regarding reported trauma mechanisms and their clinical outcome; for example, can a fall from a couch result in a femoral fracture, and if not, where is the supporting evidence?

  • Now completely revised and expanded, this second edition not only includes updated chapters detailing treatment of the most common and troublesome sites for stress fractures, but also more recent research regarding risk factors for the development of stress fractures. As in the original edition, general principles of evaluation, healing considerations and management strategies are discussed in three thematic sections. However, unlike the previous text, this edition will focus on a holistic approach to the treatment of these increasingly common injuries with new chapters on nutritional optimization of athletes at risk for stress fractures, systemic treatment strategies for healing including vitamin D supplementation and parathyroid hormone stimulation, and surgical and injectable biologic treatment modalities for enhanced bone healing. This edition will also include guidelines for on-the-field and training room management of stress fractures for team physicians. This revision is timely, given the increasing rate and severity of bony stress injuries encountered in elite and professional athletes related to single-sport specialization and the increased popularity of endurance sports such as cross-fit, obstacle course racing, and ultramarathon competitions. It will be an ideal resource for sports medicine practitioners including orthopedic surgeons, primary care sports medicine specialists, athletic trainers, and physical therapists.
